The Forward-Booking Deficit: Your Veterinary Practice Can’t Afford to Skip Scheduling the Next Appointment

When veterinary practice owners and hospital managers review their profit and loss (P&L) statements, they naturally focus on the "Big Three" expenses: cost of goods sold (COGS), inventory, and labor. However, in a shifting market that’s seeing a decline in veterinary visits5, the most significant threat to a clinic's financial health isn't always what's going out — it's the revenue that never makes it onto the books in the first place.

During Scratch’s Finding Hidden Opportunities in Your P&L webinar, hospital manager Brian Carlson, MBA, and veterinary accounting expert Mark McGaunn, CPA, CFP®, sat down to expose the "invisible leaks" quietly draining clinic profitability. At the top of their list? The veterinary industry’s struggle to master forward booking.

The true cost of a missed booking for vet practices

Forward booking is a vastly underused strategy: only 11% of veterinary practices1 book preventative care appointments in advance. Compare this to the dental industry, where forward booking is a universal operational standard. As Mark noted in “Finding Hidden Opportunities in Your P&L”, dental hygienists will practically block the exit with their chairs until your next six-month cleaning is on the calendar.

In veterinary medicine, failing to secure that next appointment before the client walks out the door sets off a damaging financial and clinical chain reaction:

  • The $250 leak: The average veterinary visit value sits at $250.392. When a single annual wellness visit is missed, that loss compounds across the entire lifetime value of the patient.
  • The compliance chain breaks: Pets quickly fall behind on necessary preventative care, booster series, and early diagnostic screenings.
  • The staffing nightmare: Labor is the single largest expense on a veterinary P&L. Without a forward-booked schedule, managers can’t accurately project future client volume, making optimized staffing schedules nearly impossible. When schedules are empty and staff are sent home early, it acts as a massive team morale killer.

The shifting timeline of veterinary care

The problem is being accelerated by an increased amount of time between clinic visits. Recent data from the VHMA reveals that the average time between veterinary visits from July 2020 and July 2021 was 72.8 days. From July 2023 to July 2024, this figure has jumped to 112.3 days — a staggering 48% increase3.

This delay means that real money is slipping away. A current VetSource report highlighted that visits were down 3.1% per practice in 2025 compared to the previous year, with a 3.8% drop in wellness visits2, indicating that clients are continuing to stretch the time between appointments. When clients are left to a purely reactive reminder system, it is even more likely they’ll delay care or silently drift to a competitor down the road.

Turning the tide on forward bookings

The solution doesn't require aggressive front-desk blockades, but it does require proactive scripting. Brian suggested a simple, empathetic approach during Scratch’s recent webinar: if a client is checking out at 11:00 AM on a Monday, the front desk team should look six months out and say, "It looks like Monday mornings work well for your schedule. I have an opening with Dr. Jones on this date; let’s go ahead and secure that for Fluffy's booster now. You'll get text reminders, and you can always modify it if your work schedule changes."

The financial incentive for implementing this system is clear. Practices that successfully forward book just 30-35% of their client base start seeing a double-digit increase in revenue4. By protecting the schedule 30, 60, 90, or 180 days out, you secure both job safety for your team and sustainable, predictable financial health for your practice.
